WesleyCare Mount Gravatt is purpose built for people with High Physical Support needs, including individuals with complex medical requirements. Our service is ideal for those who want to live independently in their own modern apartment while still having access to 24/7 onsite support whenever they need it.
We take a person-centred approach to support, ensuring that your individual needs, goals, and preferences are at the centre of everything we do. Our team works collaboratively with you, your support network, and any relevant providers (such as Support Coordinators, allied health professionals, and, where applicable, housing providers) to deliver consistent and responsive supports
Our primary funding stream for WesleyCare Mt Gravatt is the NDIS. The apartment is designed for one resident in a two-bedroom setting and will require Specialist Disability Accommodation (SDA) funding for a High Physical Support apartment.
You will also require Core Supports or Home and Living funding to access Supported Independent Living (SIL) depending on your individual needs.
We can also support people with alternative funding arrangements, such as the Transport Accident Commission (TAC) or the National Injury Insurance Scheme Queensland (NIISQ).

Our service provides shared supports at a 1:2 ratio with active overnight support, ensuring participants receive consistent assistance both day and night.
Our experienced team includes qualified Disability Support Workers (Cert III) and RN oversight, enabling us to deliver high-intensity supports for participants with complex needs. Participants are also supported to engage in community participation activities, promoting independence and social connection.
Together, this integrated model ensures each participant receives the right level of personalised care in a safe and supportive environment.
Yes, Our housing and support model combines Specialist Disability Accommodation (SDA) and Supported Independent Living (SIL) into a fully integrated service designed to provide safe, high-quality, and consistent care.
We offer modern, purpose-built homes made up of self-contained dwellings within a supportive community environment, and when you live with us, you also receive SIL supports under a separate agreement to help you live as independently as possible.
We are committed to transparency and will always explain your options clearly, including the potential conflict of interest in providing both services, so you can make an informed choice. By entering into SDA and SIL agreements with us, you are choosing a comprehensive, aligned model of housing and support, and we are committed to delivering this to the highest standard.
